Transition from high school to postsecondary options involves identifying a student’s vision for the future and the services needed during high school to realize that adult goal. This service begins at age 14, or sooner if appropriate, with a statement of ‘transition service needs’ such as a course of study. At age 16, or sooner if appropriate, a statement of ‘needed transition services’ is developed which includes: instruction, related services, personal development, community experiences, daily living skills, and interagency linkage, if appropriate. These services are intended to assist students in making the transition from the world of school to the world of adulthood. Planning the types of transition services needed will be discussed as the IEP team considers postsecondary education and employment, independent living skills, and community participation.
The Ohio Rehabilitation Services Commission (ORSC), in partnership with ODE/OEC, was awarded a 2.2 million dollar, 5 year grant to direct the work of regional Secondary Transition Specialists and Rehabilitation Transition Specialists in improving the outcomes for secondary students with disabilities as measured through Ohio’s State Performance Plan (SPP) Indicators:
Regional Transition Councils, developed in each SST region, will provide technical assistance and information dissemination to local school districts and other stakeholders from data obtained through the Secondary Transition team.
