The Ohio Department of Education’s Office for Exceptional Children asks all Ohio districts to register any attending
student who is legally blind, including preschool, homebound,
home-schooled or adult students.
Each year we contact districts to request participation in the Federal Quota
Registration of Blind Students. I am writing to let you know that the 2020
registration is now open. Please find two documents with more information about
this request attached to this email:
- 2020 Federal Quota Registration of Blind Students Letter
- 2020 Federal Quota Registration of Blind Students Registration Form
Superintendents/Directors will need to:
1. Download registration form (attached to this email)
a. Save to your desktop
2. Update and make changes to the student information
a. Changes can be typed directly into
the form
3. Save the form
4. Print and sign the appropriate line
5. Make a copy of signed registration form
a. The copy should remain
on file at the school district
b. Parental permission
form(s) will remain on file at the school district
6. Mail the original registration form to the AT&AEM Center via
the United States Postal Service. No faxed forms will be accepted.
